VCH philosophy is that home is best, as most people wish to remain in their own home for as long as is safely possible. VCH is committed to quality of life by supporting independence, health, and wellness through the delivery of person and family-centered care. Home Health provides health-care services in your home or at a Home Health clinic, based on the most appropriate care for your needs. We are dedicated to enhancing your quality of life by promoting independence, health, and wellness through personalized, family-centered care.

VCH works in partnership with Providence Health Care in Vancouver to take patients referred from across the province. Together, they provide 46 percent of all inpatient specialized care for British Columbia. If you find that your daily care needs and health issues make it difficult to continue living in your home, it may be time for you to look at a safer alternative. The decision to move into residential care is very personal. It should be made after carefully reviewing your situation, your needs and your options.

There are six main VCH service options to consider as follows:



1) VCH Convalescent care:

Convalescent care or transitional care is a short-term residential service available for people who have been in hospital and no longer require acute hospital care but who need a little more time to recover. It is provided through our Transitional Care Units (TCU). The length of stay depends on your needs but is usually less than three months.

A team of health-care professionals works together to help you regain strength and mobility so that you can safely return to your community. To help you transition back to your home or an alternate home environment, you will be expected to actively participate in recovery, including goal setting, personal care and mobility. Your care team will work with you to develop a plan to help you achieve your recovery goals.

2) VCH Home Help and Support:

Home Health provides health-care services in your home or at a Home Health clinic, based on the most appropriate care for your needs. We are dedicated to enhancing your quality of life by promoting independence, health, and wellness through personalized, family-centered care.

3) VCH Home and Community Care:

If you wish to arrange for home and community care services, the first step is to contact VCH, and they will help you prepare for an assessment, develop your care plan, and what to expect once you have been approved for services.

4) VCH Home and Community Care Services:

General eligibility:
If you or a family member want home and community care services, you must need care at home after you have been released from the hospital to prevent you from going back or for a life-limiting illness.

What to Except:
VCH Home Health services are offered to clients and/or caregivers to restore or maintain independence in the home. This may include the following clinicians as part of your care team:


    CASE (CARE) Managers


    COMMUNITY NURSING


    AMBULATORY CARE NURSING


    OCCUPATIONAL THERAPIST


    SPEECH-LANGUAGE PATHOLOGISTS


    SOCIAL WORKERS


    DIETITIANS


    PHYSIOTHERAPISTS


    SPIRITUAL HEALTH PRACTITIONERS

5) VCH Palliative Care in your home:

A palliative approach to care can be provided to you and those you choose to be involved in your care from:
- the comfort of your own home or
- Assisted Living home.

6) VCH Residential care:

Provides 24-hour professional care in a supportive environment. Trained staff assists with personal care and daily living activities, such as bathing, dressing and managing medications. Residential care also offers a variety of therapeutic and recreational activities. While services such as meals, laundry and housekeeping are provided for you, every effort is made to help maintain your independence.
